We are very blessed to have been in business for over a decade and owe this to the support of our community.  Deja Vu Med Spa’s primary charitable effort is to focus on programs supporting children, education, and health and human services in the community where we do business. Throughout the year, we receive a large number of requests and regret that we are unable to meet all requests since the need is far greater than our resources. In order to be eligible the following guidelines must be met:

1. Must be a 501(c)(3) nonprofit organization
2. Focus on supporting children, education, and or health and human services
3. Must be in a 10-mile radius of Deja Vu Med Spa or be a charity supporting by an existing client.
4. Must submit request a minimum of 30 days before the need date.
5. Submit form and 501 documentation to the email below

If you would like to request a donation, please submit a request to jdursteler@dejavumedspa.com (with charitable donation request in the subject line) and attach the following completed form no later than 30 days prior to your deadline. Requests without the proper application will not be reviewed. We thank you for your continued support.
